what is this ?
In HAZOP
analysis, an interdisciplinary team uses a systematic approach to identify
hazards and operability problems occurring as a result of deviations from the
intended range of process conditions, using a fixed set of guide words which
are applied to process parameters at discrete locations, or study nodes, in the
system.
Hazard
Identification (HAZID) is a technique for identifying, at a high level, all
significant hazards associated with the particular facility, unit or operation
under consideration. It is a systematic review of the causes and consequences
of hazardous events. HAZID can be applied to a wide range of different
facilities and is not limited to oil & gas facilities.
Safety integrity and safety integrity level (SIL) are concepts
defined in the Safety Instrumented System (SIS) safety lifecycle. SIL is a
property of a safety function rather than of a system or any part of a system,
and includes both hardware safety integrity and systematic safety integrity.
An inspection is a surface-by-surface
investigation to determine whether there is lead-based risk in a mechanical
equipment, piping, and pipeline facility, and
where it is located. Inspections can be legally performed only by certified
inspectors or risk assessors. Lead-based risk inspections determine the presence of lead-based risk. It is particularly helpful in determining
whether lead-based risk is present prior
to engineering, procuring, or construction, and identifying
potential sources of lead risk exposure at any time.
A risk assessment is an on-site investigation to
determine the presence, type, severity, and location of lead-based risk hazards
(including lead hazards in mechanical equipment and piping on tanker (ship),
offshore platform, and industries) and provides suggested ways to control them.
Risk assessments can be legally performed only by certified risk assessors.
Lead-based risk assessments are particularly helpful in determining sources of
current exposure and in designing possible solutions. We can also have a combined
inspection and risk assessment on equipment (mechanical and piping) on
different location (industries, tanker ship, or offshore platform).

who ARE the instructors ?
Muhammad Zahirsyah, MT.
He has 20 years’ experience in offshore/
onshore oil and gas, geothermal and petrochemical projects, commissioning, and
plant operations. He has a Master’s Degree from the Institut Teknologi Bandung
(ITB) and is a candidate doctoral
at Universitas Indonesia (UI), majoring in Chemical Engineering.
His
extensive experience includes project management as Project Manager and
Engineering Manager for numerous engineering design projects (conceptual
design, FEED, and detailed engineering design). He is well-versed with
international codes/ standards and specifications such as API, NFPA, ASME,
NACE, TEMA, DNV, ANSI, etc.
His broad client experience includes work executed for:
ConocoPhillips, Pertamina EP, Kangean Energy Indonesia (KEI), ENI Muara Bakau,
BV., Chevron Pacific Indonesia (CPI), PGN, PLN, Exxon Mobil, PTT Thailand,

Having 20 years of experiences in the
industry, mostly in the Maritime and Oil & Gas consulting, verification and
inspection services, with educational background as a Bachelor Engineering
degree in Naval Architecture and also holds a Magister Business of
Administration. Working with
International Classification Societies such as GL, BV, LR, and the latest
position as Indonesia Country Manager DNVGL Oil and Gas Services. Mainly
dealing with marine survey, inspection, quality management system, risk
assessment, and asset integrity Management system of offshore and onshore
assets, such pipelines, offshore platforms, and floating units.

The course
will be of use to a wide range of disciplines whether in design, development,
production, maintenance or a safety/environmental role. People in all these disciplines
regularly lead, or are involved in HAZOP/HAZID/SIL and Survey Inspection. These includes: Managers, Risk Assessors, Operators
/ Technicians who deal with HAZOP/HAZID/SIL, Cost Estimator, Project Engineer, Lead Engineer, Senior Engineer, Project Manager AND Surveyor and Inspector.
